Integrated Michigan Patient-centered Alliance on Care Transitions (I-MPACT) is Blue Cross Blue Shield of Michigan’s first collaborative that brings physician organizations and hospitals to work together. I-MPACT works with hospitals, providers, community service organizations, patients and families. I-MPACT uses standard practices and innovative processes to improve transitions of care for patients. 

This hybrid Collaborative Quality Initiative (CQI) works to improve quality and reduce variation in care transition practices among physicians and hospitals for the following target populations: patients with chronic heart failure, patients with chronic obstructive pulmonary disease, patients with pneumonia, patients with acute myocardial infarction, and patients transitioning to a skilled nursing facility. Hospitals and physician organizations work together to improve seven day follow up visits with primary care physicians, reduce readmission rates, and reduce emergency department visits.
