What’s a Health Assessment and How Do I Take It?

Our online health assessment, powered by WebMD®, is a set of questions that helps you understand how you can improve or maintain your health.

After you complete the health assessment, you'll get a customized plan to help you address any health risks you've identified.

You can get your health assessment started in two easy steps. Here’s what you have to do.

Please note that not all our plans include a health assessment through Blue Cross Health & Well-Being. If your plan doesn’t, you won’t see a link to Health & Wellness when you log in to your account.

How to take a health assessment

Step 1: Log in to your account

Your health assessment is only available through your online account with us. You can log in on our website or use the Blue Cross mobile app.

Don’t have an online account? You can register for one on our website or the mobile app. Just keep in mind, each adult taking a health assessment will need their own account.

Step 2: Select Programs & Services, then WebMD

That will take you to the Blue Cross Health & Well-Being website. Once you get there, select Resources in the left-side navigation box, then select Assess Your Health. Select the Health Assessment card, then select Take It Now in the box that pops up. 

Login tips

  • Make sure you can access your online account before your health assessment is due. You need to know your username and password. 
  • If you can’t remember your username or password, you can recover them online. Have your Blue Cross ID card and the answers to your security questions handy.
  • Are you waiting for a PIN to come in the mail after registering for an account? You don’t need that PIN to complete your health assessment. You can still log in and take your assessment without it.
