24-Hour Nurse Line

You can call Blue Cross Blue Shield of Michigan's 24-Hour Nurse Line for confidential help with questions about your health. Our nurses can help you better understand your condition or manage your symptoms. They can also help find a doctor or a support group in your area. When you need help with your health care, personal service is just one call away.

Health assessment and coaching tools

When you take your health assessment, we can help you understand and learn about your health risks so you can talk about them with your doctor. You’ll also find out about any follow-up programs or digital coaching modules available to you. Depending on your plan, you may get your health assessment in the mail or you can take it online. You'll need your Blue Cross ID card. 

All Medicare Plus BlueSM Group PPO members should take the Medicare Advantage health assessment.

Tobacco Coaching

This program is available to all UAW Trust members with a Blue Cross plan.

Are you trying to quit smoking or stop using tobacco? Tobacco Coaching, powered by WebMD®, can help. It’s a 12-week, phone-based program that connects you with a health coach. Together, you’ll work through the quitting process.

To be eligible for the program:

  • You'll need to be ready to set a quit date within the next 30 days
  • You must have used a tobacco product within seven days of your initial call to WebMD

This program is available to Medicare Advantage PPO, Enhanced Care PPO, BCN Advantage and Blue Care Network members.

If you've been hospitalized for mental health or substance use disorder, we can help. Our case managers will work with you and your doctor to prepare for when you go home. Together, we'll figure out what you'll need to continue your care.

Case management

This program is available to Medicare Advantage PPO, Enhanced Care PPO, BCN Advantage and Blue Care Network members.

If you're living with an illness, our case management program can help. We'll work together with you, your family and your doctor to improve your quality of life. The program usually lasts about three months.

Our team of registered nurses, social workers and physician consultants can:

  • Teach you and your family about your condition
  • Talk to your doctors to coordinate your care
  • Provide health coaching
  • Discuss advance directives

Nurse case managers will call you to discuss your health conditions. Together, you'll set goals and build the skills you need to manage your condition. The nurse will call back as often as you like.

If you have more than one chronic condition, we have a program for you, too. Nurse case managers will work with you over the phone to help you set goals and develop a plan to manage your condition.

You can contact 2nd.MD for a second medical opinion. 2nd.MD services are available at no additional cost to you. At 2nd.MD, you're connected with doctors across the county for an expert medical consultation via phone or video. 2nd.MD specialists can help you understand a new or chronic diagnosis and treatment options, learn about breakthrough treatments, or understand if a recommended surgery is the best option for you.

As a Medicare Advantage member, you get access to special programs to help you with your health.

Advocate Plus Program

Your Care Advocate is ready to help. The Advocate Plus program is for Medicare Advantage PPO plan only. Care Advocates can guide you on your health care journey by helping you find resources and a doctor that’s right for you. They can also help you to schedule an appointment, navigate your health care choices and find answers to general and complex questions. They’ll help you find all the information you need to improve and maintain your health.

You can connect with a Care Advocate online or through the BCBSM Coordinated Care app using the code MIAUTO.

SilverSneakers®

This program is available for Medicare Advantage PPO and BCN Advantage members.

Have fun and make friends at the gym with this fitness program. You can take fitness classes designed specifically for older adults. You’ll get access to more than 14,000 gyms across the country, plus online resources to help you lose weight, quit smoking or reduce stress. All at no extra cost.

Care transition to home

This program is only available for Medicare Advantage PPO members.

After you get out of the hospital, the last thing you want to do is go back. This program gives you the support you need to safely leave the hospital and take care of yourself at home.

During your first 30 days at home, we'll help you:

  • Get the care you need
  • Understand your medications
  • Watch out for worsening symptoms
  • Follow-up with your doctor
  • Get started with health management programs, if applicable

Landmark Health

This program is only available for eligible Medicare Plus Blue PPO and BCN Advantage HMO-POS members in select areas.

Medicare Plus BlueSM and BCN AdvantageSM are PPO, HMO, and HMO-POS plans with Medicare contracts. Enrollment in Medicare Plus Blue and BCN Advantage depends on contract renewal.

With the exception of emergency or urgent care, it will cost more to get care from non-plan or non-preferred providers. You may receive services from any provider who accepts Original Medicare. Your out-of-pocket costs will be lower if you choose a network provider. To find a network provider, visit Find a Doctor.

Limitations, copayments and restrictions may apply. You must continue to pay your Medicare Part B premium. Consult your Evidence of Coverage for specific information about your benefits and cost-sharing responsibilities.
