Within Your First 120 Days
As part of the Healthy Blue Living program, you may need to complete additional steps if your Blue Care Network Qualification Form comes back with certain indicators.
Why 120-day to-do's?
If your qualification form shows you use tobacco or you have a BMI of 30 or higher, you choose what's next to maintain your enhanced benefits and keep your costs down. Your plan comes with Blue Cross Well-Being that has weight-management and tobacco-cessation programs and resources to help you meet your 120-day Healthy Blue Living to-do's.
Maintaining your enhanced benefits
In the first 120 days — if required:
- If your BMI is 30 or higher, enroll and start participating in a weight-management program
- Weight-management options include a Triple Tracker or Lifestyle Coaching program
- If your qualification form shows you use tobacco, enroll and start participating in our tobacco coaching program
