What Continuity of Care means

Continuity of Care is a program to help pay healthcare costs for certain medical conditions by providing extended time to complete a course of treatment and/or transition care. Members identified as eligible may continue active treatment for an approved Continuity of Care condition within the Michigan Medicine system for up to 90 days from July 1.

Who may qualify

Members undergoing a course of treatment for a complex or time-sensitive condition may qualify.

Examples may include:

  • Active cancer treatment, such as chemotherapy, radiation or targeted therapy
  • Recent surgery requiring ongoing follow-up care
  • Organ transplant evaluation or recovery
  • Pregnancy with an established treating provider
  • Behavioral health treatment with active medication management
  • Complex or life-threatening conditions where interrupting care could be harmful

Routine monitoring for a stable condition may not qualify.

What to do next

Eligibility is determined through clinical review by a Blue Cross physician. 

Members who qualify for Continuity of Care will receive a letter or email with details about next steps.
