Advocate Overview

Blue Cross Coordinated Care Advocate is our intermediate care management option that provides access to personalized clinical care management for all your employees and covered family members, and greater engagement of those with complex and chronic conditions. It builds upon Coordinated Care Core and Coordinated Care Enhanced. 

 

The Advocate offering includes access to Care Advocates who provide all of your employees clinical guidance and episodic support. With the support of Nurse Care Managers and Care Advocates, your employees will be more confident in their ability to find the care that’s right for them and their plan.

What can Advocate do for your employees?

Here's what you can expect:

  • Reduced dollar threshold for claimant reviews, lowered to $75,000 from $100,000, meaning a greater number of your employees will have access to care management
  • One-on-on support from care advocates who provide your employees with personalized clinical support to address health care needs
  • Access to the BCBSM Coordinated Care app which gives all your employees and their family personalized recommendations, simple connections to care teams and curated health and benefit content to support their needs
  • A Member Snapshot that provides Blue Cross with a comprehensive, personalized view of each member using a cloud-based data and informatics infrastructure

Coordinated Care Advocate features

These features include:

  • A holistic, member-centric approach to member support
  • Comprehensive support across medical, behavioral, pharmaceutical and social needs
  • Coordination with local providers and community resources
  • An easy-to-use app to help employees get answers for all their questions
  • Enhanced analytics to identify and prioritize the most impactable members 

Care Advocates support your employees with personalized clinical support helping to guide them along their health care journey. Care Advocates provide clinical guidance and episodic support for your employees by assessing short-term needs, coaching them on health care related issues, and proactively working to resolve potential barriers to care. They may also guide your employees to appropriate levels of care and provide interventions to assist in resolving gaps in care. 

With the support of Nurse Care Managers and Care Advocates, your employees will be more confident in their ability to find and seek care, providing long-term value to your company and your employees. 

Through the clinical engagement that comes along with the Core and Advocate offerings paired together, your employees will be more in-tune with their health care than ever before. That means better health and financial outcomes for your company and your employees. 

As a step up from the Enhanced offering, Advocate includes reviews from our high dollar claims team for claims of $75,000 (the minimum for Core is $150,000 and for Enhanced is $100,000). We will review these claims to ensure that every dollar spent is spent wisely, saving both you and your employees money while also improving how your employees find a get care.
