How to report fraud

You may remain anonymous when reporting health care fraud. All information Blue Cross Blue Shield of Michigan receives is strictly confidential. But if you'd like a response from us about your report, please include your contact information.

We maintain a policy that enforces non-retaliation and non-intimidation against those who report potential concerns.

You can use this online form to file a fraud report. Please note that the information you submit isn't secure or encrypted. You can call us or mail in a report if that concerns you.

Through the mobile app

Members can also report fraud through the Blue Cross Blue Shield of Michigan mobile app. Select Contact Us.

By phone

You can call us 8:30 a.m. to 4:30 p.m. ET, Monday through Friday. TTY users can reach these numbers by calling 711.

After hours calls will be recorded. A fraud specialist will return your call the next business day to collect further information.

By mail or fax

Download, print and return the fraud complaint form (PDF). Please include your contract number and the date of service the complaint is about. The mailing address and fax number are on the form.
