- If you have a social security number (SSN) and provided it when you enrolled, log in to your online account and choose "Forms." You'll be able to download a copy of the form. If you didn't provide your SSN when you enrolled, you won't be able to download a copy of the form, but you can use option 2 or 3.
- Call the phone number on the back of your enrollee ID card.
- Mail your request to Blue Cross Blue Shield of Michigan, 600 E. Lafayette Blvd., Detroit, MI 48226.

How does it work?
All currently enrolled U-M students on the Ann Arbor campus who pay the health service fee are eligible for services at University Health Service and don’t pay the office visit copay when seen at UHS. Ann Arbor students who wish to seek care outside of UHS and Flint and Dearborn students can check below under “Find a doctor."
