The ConnectedCare option is a custom partnership with Henry Ford Health. This partnership delivers primary and specialty care, walk-in/urgent/emergency care, and hospitalization health care services to members from providers and facilities located throughout the Detroit area that are a part of the Henry Ford Health network. This includes more than 3,000 doctors and advanced practice providers in 40 specialties. Additionally, DMC Children’s Hospitals, specialty centers and specialty providers delivering services for pediatric specialties not available through Henry Ford Health are included in this ConnectedCare partnership.
Level 1. Providers participating in the ConnectedCare network are called Level 1 providers and covered services pay at Level 1 benefits.
Level 2. Providers and facilities that are not a part of the ConnectedCare network but participate in the broad Blue Cross Blue Shield of Michigan (BCBSM) PPO network are called Level 2 providers. Covered services received from these providers and facilities pay at Level 2 benefits.
ConnectedCare provides lower out-of-pocket costs when you

To be offered in a particular zip code, there must be adequate coverage to treat both adult and pediatric health needs in every specialty. For each specialty type, the partnership ensures enough providers and facilities available to furnish health care services across a broad population. Henry Ford Health is committed to expanding the number of providers participating in this partnership, with a strong focus in the Southeast Michigan, tri-county area.
Emergency care for qualified medical emergencies is covered at any hospital.
If your dependent child lives outside of the Detroit area (e.g. away at college), they should have an established patient relationship and receive their annual physical with a ConnectedCare Level 1 primary care provider. The dedicated Henry Ford Health Concierge Team can assist in scheduling appointments when your dependent is home visiting with you.
If your dependent requires primary care (prevention, wellness, and treatment for common illnesses) while away, they should contact the MyCare Advice Line (additional details below under Membership) to speak directly with a registered nurse or connect with their provider through the many virtual options available. Care received at urgent care facilities outside of the metro-Detroit Henry Ford service area are covered at Level 1.
Should your dependent child require ongoing care for a diagnosed condition while they are living outside the Detroit area, their ConnectedCare Level 1 provider will work with them to find a Level 2 provider that participates with Blue Cross Blue Shield and the Henry Ford Health Concierge Team will facilitate a waiver process (see details below) so that services can be covered at Level 1 benefits, resulting in lower out-of-pocket costs for you.

This includes more than 3,000 doctors and advanced practice providers in 40 specialties. Additionally, DMC Children’s Hospitals, specialty centers and specialty providers delivering services for pediatric specialties not available through Henry Ford Health are included in this ConnectedCare partnership.
Level 1. Providers participating in the ConnectedCare network are called Level 1 providers and covered services pay at Level 1 benefits.
Level 2. Providers and facilities that are not a part of the ConnectedCare network but participate in the broad Blue Cross Blue Shield of Michigan (BCBSM) PPO network are called Level 2 providers. Covered services received from these providers and facilities pay at Level 2 benefits.
ConnectedCare provides lower out-of-pocket costs when
you receive care from ConnectedCare Level 1

To be offered in a particular zip code, there must be adequate coverage to treat both adult and pediatric health needs in every specialty. For each specialty type, the partnership ensures enough providers and facilities available to furnish health care services across a broad population. If your dependent child lives outside of the Detroit area (e.g. away at college), they should have an established patient relationship and receive their annual physical with a ConnectedCare Level 1 primary care provider. The dedicated Henry Ford Health Concierge Team can assist in scheduling appointments when your dependent is home visiting with you.
If your dependent requires primary care (prevention, wellness, and treatment for common illnesses) while away, they should contact the MyCare Advice Line (additional details below under Membership) to speak directly with a registered nurse or connect with their provider through the many virtual options available. Yes. Emergency care for qualified medical emergencies is covered at any hospital at Level 1. If you receive non-emergency care in the state of Indiana or anywhere else in the U.S. and its territories, you’ll be covered at Level 2. Level 1 costs less than Level 2.

We encourage all members to see their primary care physician for guidance on specialty referrals. This approach can save time and money since many health concerns can be addressed by a primary care physician.
However, you don’t need a referral to see a specialist in HealthSync. Given the scope of HealthSync, there should be very few other instances in which care isn’t available within HealthSync. If you can’t find a specialist, your HealthSync physician will help you get the care you need at Level 1.
