If you're enrolled in a Medicare Advantage plan, we'll send you a yearly letter about taking your health assessment.
The letter will have directions and a link to complete the health assessment online. You can also log in to your online member account to access your health assessment.
The letter will explain that you can either fill out an enclosed paper survey or answer the same questions online. If you choose to fill out the paper survey, remember to mail it back to us.
