Clinical Formulary

The Clinical Formulary (PDF) provides coverage for all covered drugs, regardless of whether they are listed on the formulary. However, certain drugs on the formulary may also require prior authorization or step therapy. 

In addition, some drugs, such as those intended for cosmetic use, may be excluded from coverage.

Formulary Updates

The Custom Formulary listing is updated twice a year. Since a drug's status may change and new drugs may be added to the formulary, we also publish a list of formulary updates to keep you informed of those changes.
