Who is this for?
If you're a Blue Cross Blue Shield of Michigan or Blue Care Network member, this information can help you learn how to make changes to your health insurance plan information.
We’ll always be sure to let you know about changes to your health care benefits. If anything changes with your account, we’d like you to let us know, too. That way, you’ll be sure to get all the coverage you’re entitled to.
Please let us know within 30 days of any changes to your information like:
- Adding someone to your plan
- Beginning a COBRA plan
- Changing your name or address
- Changes to your contract
- Cancelling or reinstating your policy
- Making changes to your health spending account
If you’re a Blue Cross Blue Shield of Michigan member
Employer plans: If you are covered under a plan through your employer, you’ll need to fill out the New Enrollment/Change of Status form.
Individual plans: If you have a individual health insurance coverage for you or your family, you can fill out the Individual Billed Member Change of Status Request form.
If you’re a Blue Care Network member
Employer plans: If you are covered under a plan through your employer, you’ll need to fill out the New Enrollment/Change of Status form.
Individual plans: If you have individual health insurance coverage for you or your family, you can fill out the Blue Care Network Individual Change of Status form.
