If you want to see if a medication is covered by your plan, check one of these drug lists, also called formularies.
A drug list is a list of drugs approved by the U.S. Food and Drug Administration and covered by Blue Cross Blue Shield of Michigan and Blue Care Network. These medications are selected because they’re safe, effective and lower in cost.
You can use drug lists to see if a medication is covered by your health insurance. You can also find out if the medication is available as a generic, needs prior authorization, has quantity limits and more.
If your health plan begins on or after Jan. 1, 2014
And you have a:
- Individual and family plan (you buy your own health insurance)
- Small group employer-sponsored plan (you're insured through your employer, who has 50 or fewer full-time employees)
