If you have a Blue Cross Blue Shield of Michigan or Blue Care Network plan, you can take advantage of Blue Cross® Health & Well-Being. It gives you access to a wide variety of programs and resources to help you improve your well-being.

You'll find a variety of online tools and information by logging in to your account at Just click the Health & Well-Being tab, then WebMD® to enter the Blue Cross Health & Well-Being site, powered by WebMD.

Other available programs

24-Hour Nurse Line. A registered nurse health coach is available to you 24 hours a day, seven days a week if you have health questions, want information to help you make a medical decision or need support with managing a chronic condition. Call 1-800-775-BLUE (1-800-775-2583) to speak to a nurse health coach.

Case Management. Diagnosis of a serious illness can be stressful and confusing. Through BlueHealthConnection, a personal case management professional works with your doctors to help you and your family understand your condition. Being well-informed can help you make smarter decisions about your care. That, in turn, can make a real difference in your recovery. This short-term program is part of your health plan and doesn't cost extra.

Your dedicated nurse professional will work with you, your family and your treating physician to:

  • Coordinate your care
  • Inform you about your disease and treatment options
  • Provide information on Blue Cross doctors and hospitals
  • Refer you to community resources that provide emotional, financial and other support services
  • Facilitate the delivery of medical supplies and equipment prescribed by your doctor
  • Guide and support you with end-of-life issues

Chronic Condition Management. Blue Cross® Health & Wellness gives you the information and assistance you need when you’re living with:

  • Asthma
  • Diabetes
  • Chronic obstructive pulmonary disease
  • Congestive heart failure
  • Coronary artery disease

Chronic Condition Management is a short-term, voluntary program. It is part of your health plan and doesn't cost extra.



A program that encourages you to take just a few simple steps to manage your health.

Complete an annual health assessment

A health assessment gives you a snapshot of your overall health by looking at your nutrition and exercise habits.  

Select a primary care physician

LivingWell encourages members to establish and maintain a relationship with a primary care physician. A primary care physician will help you manage your health by keeping track of your health information and progress. If you don’t have a primary care physician, use to find one. 

Visit your doctor once a year for an annual checkup

A preventive visit to your doctor is the best way to stop health issues in their tracks. Your doctor will check for symptoms of disease, help you manage any chronic conditions, and discuss any questions or concerns you may have.  

Choose a Patient-Centered Medical Home 

Working with a primary care physician who is a Patient-Centered Medical Home doctor provides many advantages, including:

  • 24-hour access to your medical team
  • Coordination of care
  • Personalized strategies for managing your health
