Your Living Well Summary can keep you in control

MyLiving Voice can help you use your existing Living Well Summary to create an Advance Directive. An Advance Directive:

  • Relieves your loved ones of medical decisions
  • Avoids possible disagreement about your wishes
  • Keeps you in control of your health care

Taking this important step helps you prepare for end-of-life and unplanned medical emergencies. You can define your values, determine your care goals and convey your treatment preferences. 

If you’re eligible for My Living Voice, you should have received an email inviting you to participate. To activate your account, log in with the email address where you received your invitation and use the password your Vital Decisions Specialist gave you. Your Living Well Summary will already be in your account.
