Also known as: continuation of care

Continuity of care is when your doctor chooses to temporarily care for you under a new arrangement because the contract between your doctor (or hospital) and your health care company is terminated before your treatment is complete. Enrollment in the continuity of care program is not automatic – your doctor must initiate it and follow certain requirements set by your health carrier to participate in the program.
