Dental EOBs look different than medical statements but share some of the same information. On the first page you’ll find:

• Information about you—your plan and the patient
• Information about us—our phone number and address

Then you’ll see a Summary of Services that includes:

• The services you got, when you got them, and the dentist who provided them
• Plan balances like your deductible
• Why a service may not have been covered
