You get this form if you bought your health insurance directly from us — not through healthcare.gov — or if your Blue Cross coverage is through your employer.

Here's what else you need to know:

  • It's proof you met the Affordable Care Act requirement for the months you had coverage.
  • If you bought your coverage from us and anything on your 1095-B is incorrect, call the number on the back of your Blue Cross ID card.
  • If your coverage is through your employer and anything on your 1095-B is incorrect, contact your employer.
  • You don’t need to wait for your 1095-B to file your tax return, although you should save it for your records.

For Blue Cross members with an online account

You can download a PDF of your 1095-B form from your online account. Subscribers — the person whose name is on your Blue Cross ID card — can log in to their account and:

  • Select the "off" toggle for your Form 1095-B for Health Coverage section to stop receiving mailed copies of your 1095-B form.
  • Select "Forms" to view, download and print your 1095-B forms.
