Each person on your plan who selects a primary care physician in a Level 1 OSC gets Level 2 cost share for:

• Visits to in-network physicians and hospitals outside your selected OSC without a referral from your primary care physicians

Each person who doesn't select a primary care physician in a Level 1 OSC pays Level 2 cost share for:

• Visits to any in-network physician or hospital
