We may bill your prescription drug under your medical coverage if it's given to you:

• In your doctor’s office
• At a hospital outpatient facility
• In your home
• As an injection
• Through an infusion, or IV

If you have a Blue Cross Blue Shield of Michigan PPO or Blue Care Network HMO plan, our Prior Authorization Medical Coverage Drug List (PDF) shows the drugs under your medical coverage that we need to approve before you receive them.

Some drugs that fall under your medical coverage also have limits on the quantity that's allowed. Our Quantity Limits for Medical Coverage Drugs (PDF) outlines these drugs and the limits that apply.

If your drug is on this list, talk to your doctor about getting preapproval for it. This list doesn’t apply to Medicare plans.
