There are multiple ways to let us know if you, your spouse or any of your covered dependents have other group healthcare coverage from an employer. You don't need to tell us about Medicare or health coverage through your auto insurance policy.

Before letting us know about coordination of benefits, you will need to gather the following documents:

  • ID cards from all other health insurance plans.
  • Full name and birth date for each person covered by other insurance.
  • If someone on your plan has other insurance because of a court order from the Friend of the Court or a divorce decree, you’ll need those legal documents.

Once you have the documents, there are two ways to let us know your additional coverage.

If you are a Blue Cross Blue Shield of Michigan member, you can also mail or fax the Coordination of Benefits form (PDF)​ to us. Blue Care Network members must complete the form online.
