Who is this for?

Group Health Insurance — Learn More

If you're considering one of our HMO plans in the PCP Focus network, this explains what you need to know about this cost-saving custom network.

PCP Focus is a custom primary care physician HMO network offered by Blue Care Network. The doctors your employees will choose from this network have shown they can provide quality care and a high level of efficiency that lowers health care costs. We pass those savings on to you: your premiums are 6-9 percent lower when you choose a plan with the PCP Focus network.

Where it is

The PCP Focus network is available with our BCN HMO℠, BCN HRA HMO℠ and BCN HSA HMO℠ plans in the following counties:

  • Bay
  • Calhoun
  • Clinton
  • Eaton
  • Genesee
  • Ingham
  • Kalamazoo
  • Kent
  • Livingston
  • Macomb
  • Monroe
  • Muskegon
  • Oakland
  • Ottawa
  • Saginaw
  • Shiawassee
  • St. Clair
  • Van Buren
  • Washtenaw
  • Wayne

Your business needs to have at least one location in these counties to be eligible for the PCP Focus network. If you have multiple locations, the ones in counties outside the PCP Focus network will be enrolled in our main HMO network.

How it works

All employees and dependents must select their primary care physician from the PCP Focus network.

Finding a PCP Focus doctor is easy. Here's how:

  1. Go to Find a doctor at bcbsm.com.
  2. Select Blue Care Network PCP Focus Network (HMO) as your health plan.
  3. Search for a doctor in the PCP Focus network.

Employees have more than 4,000 PCP Focus primary care physicians to choose from. Their primary care physician can then refer them to more than 19,000 specialists around the state. Employees can go to more than 100 hospitals, too.

They can call the number on the back of their ID card if they need help finding a PCP Focus doctor.
