If you have a chronic condition, you may qualify for Blue Cross Coordinated Care Core, a program that centers on your specific needs.

Blue Cross Coordinated Care Core

Blue Cross Coordinated Care Core is a member-centered, care management program that takes a whole-person approach to help members achieve their health goals. A nurse care manager leads a team of professionals who help members develop a care plan to better manage their health. 

To meet the member's specific needs, each care team includes the following available support staff:

  • Social workers
  • Registered dietitians
  • Pharmacists
  • Medical directors
  • Non-clinical support staff

The care team communicates through a variety of methods and can include phone calls, secure messaging through our mobile app and email. Interactions can range from longer conversations, to sending text-like reminders or educational materials. The program includes a free mobile app, powered by Wellframe, to help with a convenient and easy way to stay on track, communicate with staff, set medication reminders and learn more about ways to manage health and wellness.

The care team regularly checks-in to provide education about managing conditions, coordinating care such as may be needed after discharge from a hospital and to answer health questions. Landmark Health – High Intensity In-Home Care

Landmark Health delivers in-home medical services to members with multiple chronic conditions to help them stay healthy at home and reduce avoidable emergency department visits and hospital admissions.

A key element of the program is Landmark’s coordination with the member’s existing primary care provider and specialists. Landmark is available by phone 24/7.

This program is only available for eligible Medicare Plus BlueSM or BCN AdvantageSM members in select areas.

Landmark Health will contact eligible members directly by mail and phone.
