Homeward Health now available to additional Medicare Advantage members
In the November 2024 issue of The Record, we reported that Blue Cross Blue Shield of Michigan began working with Homeward Health to provide services for Medicare Plus Blue℠ members in Michigan’s Upper Peninsula. This work started on Oct. 1, 2024.
On Jan. 1, 2025, the following additional Blue Cross and Blue Care Network Medicare Advantage members became eligible for Homeward:
- Medicare Plus Blue members in the northern Lower Peninsula
- BCN Advantage℠ members in both the Upper Peninsula and the northern Lower Peninsula
Note: Members attributed to health care providers who are in Blueprint contracts aren’t eligible for Homeward.
This change means that Homeward is now available to all our Medicare Advantage members in:
- All counties in the Upper Peninsula
- The following counties in the Lower Peninsula:
- Alcona, Alpena, Antrim, Arenac, Bay, Benzie, Charlevoix, Cheboygan, Clare, Crawford, Emmet, Gladwin, Grand Traverse, Iosco, Isabella, Kalkaska, Lake, Leelanau, Manistee, Mason, Mecosta, Midland, Missaukee, Montmorency, Newaygo, Oceana, Ogemaw, Osceola, Oscoda, Otsego, Presque Isle, Roscommon and Wexford
As we previously announced, Homeward focuses on provider collaboration and communication, and its goal is to supplement the services provided by primary care providers to improve patients’ health.
Homeward’s mission and business model are aligned to address the challenges that are unique to patients and providers in rural communities.
Homeward Health is an independent company contracted by Blue Cross Blue Shield of Michigan to provide personalized health care services to select Medicare Advantage members.
