Well-managed chronic conditions can help members avoid ER visits or hospital stays

This is part of an ongoing series of articles focusing on the tools and resources available to help FEP members manage their health.

When patients have access to resources and support to help manage a chronic condition, such as diabetes, hypertension or cardiovascular disease, the chances of an emergency room visit or inpatient hospital stay may be reduced.

Here are resources from the American Heart Association to help members manage a chronic condition:

The Blue Cross Coordinated Care℠ program is also available to provide education and support from a care manager to members with complex or chronic conditions. Blue Cross and Blue Shield Federal Employee Program® members can request enrollment by calling 1-800-775-2583.

Additional programs for FEP members include:

  • Hypertension Management Program: Members with hypertension may receive a free blood pressure monitor every two years. If you have questions, call Customer Service at 1-800-482-3600.
  • Diabetic Meter Program: Members with diabetes may obtain a free glucose meter kit every year. Members can call 1-855-582-2024 to order a free glucose meter kit.
