Additional medical benefit drugs to require prior authorization for some commercial membersWhat you need to know For dates of service on or after May 24, 2021, you’ll need to submit prior authorization requests to AIM Specialty Health® for Herceptin Hylecta™ (trastuzumab and hyaluronidase‑oysk), Herzuma® (trastuzumab‑pkrb), Ogivri™ (trastuzumab‑dkst) and Trazimera™ (trastuzumab‑qyyp) for some commercial members. Blue Cross Blue Shield of Michigan will add prior authorization requirements for additional drugs covered under the medical benefit for UAW Retiree Medical Benefits Trust non‑Medicare members. For dates of service on or after May 24, 2021, submit prior authorization requests to AIM Specialty Health® for these drugs:
