Submitting a corrected claim

In April 2014, we began using the revised CMS-1500 form, version 02/12. This form replaced both the old CMS-1500 (version 08/05) and the Status Claim Review Form.

The new CMS-1500 form changed the process for submitting corrections for claims billed incorrectly. The Status Claim Review Form is no longer needed; the original claim can easily be corrected following the steps in your provider manual, which you can find on web-DENIS.

Following are the steps for correcting claims using web-DENIS.

  1. From web-DENIS, click on BCBSM Provider Publications and Resources.
  2. Click on Provider Manual.
  3. Click on Provider Type and select yours from the “Make Selection” box.
  4. Click on the Search button.
  5. Click on the Claims chapter.
  6. Click on Correcting Billing Errors.

Please follow these steps to submit corrected claims for billing errors and to add documentation when needed. Provider Inquiry service representatives will help you with the process if you are not yet familiar with it; however they cannot make the corrections.
