Report new code for prehospice counseling

Please report new revenue code 0691 (“Pre-hospice/Charge”) when billing for Phase I prehospice counseling services.

The new code is effective Jan. 1, 2014, for a maximum of 28 visits and specifically describes Phase I prehospice counseling services. These services must now be billed with revenue code 0691 instead of 0650.

As communicated in previous issues of The Record, Blue Cross Blue Shield of Michigan expanded the hospice benefit to include Phase I counseling services, effective July 1, 2009. At that time, an assigned revenue code specific to Phase I prehospice counseling services was not available, so hospice providers were asked to use revenue code 0650 (“Hospice Service/General Classification”) to report Phase I services.
