Reminder: Submitting physical therapy claims for GM UAW hourly workers

As you may know, TheraMatrix administers outpatient physical therapy services for General Motors UAW hourly employees with Blue Cross Blue Shield of Michigan Traditional Care Network coverage. Covered physical therapy evaluation and treatment includes CPT codes *97001, modalities *97010 to *97799, and revenue codes 421 and 424 in approved office and outpatient settings.

Claims for these members should be submitted to:

All inpatient facility physical therapy and inpatient or outpatient occupational or speech therapy services are excluded from this program with TheraMatrix. BCBSM will process claims for such services and also for physical therapy services received in the emergency room or during an observation stay.
