Better glucose control available through Hygieia insulin management Providers who have patients with uncontrolled or advanced Type 2 diabetes can refer them to Hygieia for insulin management. Hygieia’s d‑Nav® insulin management program helps people get the right dose of insulin each time they inject, which helps to lower A1c levels. This is a specialized program just for insulin management and serves as part of the patient’s overall care team. Hygieia’s d-Nav program helps practices meet HEDIS goals for A1c frequency and control. Studies have shown that 90 percent of patients using the d-Nav program experience significant improvement in blood glucose levels within 90 days of starting the program. The d-Nav program automates insulin dose prescribing using an FDA‑cleared mobile app. The program includes clinical care support either virtually, or in a Hygieia clinic. The app works by tracking and analyzing each patient’s blood glucose patterns and, based on those patterns, automatically determining how much insulin is needed at the time of injection.
