Your annual deadline to save is approaching

Don’t forget that you can save $2,400 toward your annual premium when you and your spouse complete your annual physical exams and submit your forms by Sept. 30, 2023.

Remember, your annual checkups help identify issues before they become medical concerns. 

Download the 2023 PAG Preventive Care form and ask your doctor to complete and sign it at your appointment. This is the proof of your visit.  If you need to find an in-network primary care provider, log in to your member account to find one.   

To qualify for $2,400 savings  

  • You and your spouse’s annual physicals must have occurred from Oct. 1, 2022, through Sept. 30, 2023.  
  • Both of your signed forms or copies of your Explanation of Benefits statements* must be submitted by Sept. 30, 2023.

How to submit your forms 

Submit your form or EOB* to your local Human Resources department. You can also contact them with questions about the form or the Penske 2023 Wellness Program.

*If you already had your physical on or after Oct. 1, 2022, you can get copies of your statement from your online member account.
