Your annual deadline to save is approaching

Don’t forget that you can save $1,200 toward your annual premium when you complete your annual physical exam and submit your form by Sept. 30, 2023.

Remember, your annual checkup helps identify issues before they become medical concerns. 

Download the 2023 PAG Preventive Care form and ask your doctor to complete and sign it at your appointment. This is the proof of your visit.  If you need to find an in-network primary care provider, log in to your member account to find one.   

To qualify for $1,200 savings  

  • Your annual physical must have occurred from Oct. 1, 2022, through Sept. 30, 2023.  
  • Your signed form or copy of your Explanation of Benefits statement* must be submitted by Sept. 30, 2023.

How to submit your form 

Submit your form or EOB* to your local Human Resources department. You can also contact them with questions about the form or the Penske 2023 Wellness Program.

*If you already had your physical on or after Oct. 1, 2022, you can submit your statement showing the service date. Get a copy of your statement from your online member account.
