If you want to access all available info about a patient, you'll need to start by entering the facility code we assigned to you when you enrolled. If you don't have your facility code, you can still get benefits and eligibility.

Then you'll need to enter your specialty, the patient's contract number, date of birth and the first five letters of their first name.

Facilities that should use this phone number include:

  • Hospital (inpatient or outpatient)
  • Home health care
  • Skilled nursing facility
  • Substance abuse
  • Mental health
  • Ambulatory surgical center
  • Outpatient freestanding physical therapy
  • Hospice
  • End stage renal disease
