Healthy Blue Outcomes

Healthy Blue Outcomes℠, Blue Cross Blue Shield of Michigan‘s new outcomes-based PPO product, ties a financial incentive, or Healthy Reward, to positive behavioral outcomes.

In order for a member to receive their Healthy Reward, their physician must complete the qualification form (PDF), which will be replacing the Physician Attestation Form.

If a member has a medical condition that makes it unreasonably difficult or inadvisable for them to meet the health measure requirements, they may qualify for a medical waiver (PDF). If the member does qualify for a medical waiver, their physician must complete the Medical Waiver Follow-Up Form (PDF) and return it to Blue Cross Blue Shield of Michigan within 210 days of the benefit start date.
