How do I add providers to my existing web-DENIS ID?

To add additional medical provider codes to your existing web-DENIS ID, complete the Authorization for Additional Provider Codes application (PDF).


To add additional dental provider codes to your existing web-DENIS ID, complete the Dental Authorization for Additional Provider Codes application (PDF).






I am a billing service, how do I add providers to my existing web-DENIS ID?

Complete Addendum "B" Authorization for Representative Access (PDF) to add provider codes to your existing web-DENIS ID.




Can I reassign a web-DENIS ID that's no longer being used?

Yes. To reassign a web-DENIS ID that is no longer being used, list the following information on your company letterhead (fax coversheet is not acceptable):


  • The user ID
  • The previous user's name
  • The new user's name
  • Telephone number for the new user
  • Contact person name and e-mail address
  • A short note explaining that you would like the ID reassigned
  • Signature and title from a manager, provider or authorized individual with legal signing authority.

Fax the information to 248-486-2345. We'll contact the new user within five business days.
