Enrollment

To enroll with BCBSM and/or BCN networks, select your classification from the list below. Physicians and Professionals will be presented a few questions which will determine the proper forms required to complete your submission.

Please Note: Independent Physical Therapists are now able to enroll as Groups. Please check the box "Enroll a New Group" to see the applicable Group Enrollment documents.

Registering with CAQH

All physicians and other individual allied health practitioners must also complete an application with the Council for Affordable Quality Healthcare (CAQH) Universal Credentialing Datasource (UCD) system. Blue Cross Blue Shield of Michigan and Blue Care Network will use the information you submit to complete your enrollment or change request, reducing the time and hassle of paperwork.
