I am a Healthy Blue Outcomes member; will I receive the Healthy Reward with the lower cost share?
During your initial enrollment into the Healthy Blue Outcomes plan, you will begin with the Healthy Reward cost share savings for the first 90 days of your benefit year.
In order to maintain the Healthy Reward, you and your covered spouse must meet the following three requirements each year by specified deadlines:
If you and your covered spouse don't meet the health measures this benefit year, you may qualify for the Healthy Reward next year.

I don't own a computer. How do I fulfill the qualification form and health assessment requirements?
You will receive a printed qualification form in your member welcome kit. Once you and your doctor complete the form, you can fax it to the telephone number listed on your qualification form.
If you do not have access to a computer to take the online health assessment, call 1-800-873-0509 to request a paper version.

If I do not meet the health measures, but I agree to make health improvements, will I be eligible for the Healthy Reward? For example, if I agree to quit smoking or participate in a weight management program, will I receive the cost share savings?
You must meet the required health measures within the first 90 days of your plan's effective date to receive the Healthy Reward. If you do not meet the health measures, you have a full year to work toward improving your health so that you qualify for the Healthy Reward the following year.

What if I meet the health measures, but my spouse does not?
Both you and your covered spouse must be compliant to receive the Healthy Reward. This means that you and your covered spouse must both complete the online health assessment, submit the qualification form and meet the health measures. If either you or your spouse fails to do any one of the required items, then you will not be eligible for the Healthy Reward.

How will I receive the qualification form?
A Healthy Blue Outcomes welcome kit will be mailed to you at the beginning of your benefit year. It contains the qualification form along with details of the plan requirements. This form is also available in Member Secured Services for printing.

What if my physician doesn't feel like I can meet the requirements because of a health condition?
You can request a medical waiver if you and your physician feel that you can't meet the health measures due to a medical condition that makes it inadvisable or unreasonable to attain the Healthy Blue Outcomes standards. During your office visit to complete the qualification form, request that your physician complete a medical waiver. Your physician will have access to a Medical Waiver Request Form.
Once completed, submit the Medical Waiver Request Form with your qualification form. Fax both forms to the telephone number listed on your qualification form.

Is my initial doctor's visit to complete the qualification form subject to the cost share?
Your initial visit to complete the qualification form for the health measures assessment, including necessary laboratory tests, will be covered at 100 percent as part of the preventive benefit.

Yes, these services are subject to the cost share.
