Clinical Formulary
Our Clinical Formulary (PDF) provides coverage for all covered drugs, regardless of whether they are listed on the formulary. However, certain drugs on the formulary may also require prior authorization or step therapy.
In addition, some drugs, such as those intended for cosmetic use, may be excluded from coverage.
If you're not sure whether your plan uses the Clinical Formulary, check your prescription drug benefit packet or contact customer service.
Formulary updates
The Clinical Formulary listing is updated twice a year. Since a drug's formulary status may change throughout the year, we also publish a list of formulary updates to keep you informed.
