Master Medical Claim - Inquiry Online

If you use our online Master Medical inquiry, you will be able to submit one question for each inquiry. Once you have completed an inquiry, you will receive instructions on how to submit additional Master Medical-related questions. You will only have to enter your subscriber information once.

What You Will Need to Submit an Inquiry

  • BCBSM ID Card
  • Explanation of Benefits statement

If this inquiry concerns a spouse or other covered dependent, you will also need:


  • Name of patient
  • Birth date
  • Claim number
  • Date and type of service
  • Original charge
  • Name of doctor or facility
  • Contract number for other medical coverage (if applicable)
