  • Your contract number as shown on your ID card
  • Your daytime phone number, including your area code
  • Patient's name and name of health care provider
  • Date that the service in question was performed

Always keep copies of bills, forms, letters, or receipts you send us. Never send us the original because we are unable to return them.
