What is Medication Therapy Management?

If you take a lot of medications, you might qualify for a program that helps you manage your prescriptions. Learn more about it here.
When you have chronic health conditions like arthritis or diabetes, prescription drugs can be a big part of your life.
The Medication Therapy Management Program helps make sure you’re using the right drugs for your conditions. It also helps us identify medication errors.
If you qualify for this program, our team of pharmacists and doctors will work with you one-on-one. They’ll make sure you get the best results from the drugs you take.
You qualify for this program if you:
- Have at least two chronic health conditions.
- Take six or more prescription drugs each month.
- Spend at least $3,017 on prescriptions in a year.
While this program isn't a benefit, it's included in all our Medicare plans that include prescription drug coverage. Your enrollment is free, voluntary, and doesn’t affect your drug coverage under Medicare.
How do I sign up?
If you qualify for this program, we’ll reach out to you and ask you to fill out a Personal Medication List and send it to us for review. You can find the Personal Medication List in our forms section.
Once we get your completed list, a pharmacist will look it over. If they have any concerns, they’ll call you or your doctor.
