If you have one of our Medicare Advantage plans, this information explains the programs we offer to help you manage chronic conditions.

If you're dealing with a chronic illness, like diabetes or heart disease, managing your condition is a good way to maintain or improve your quality of life.

We want to help you understand your condition and feel in control again. To do that, we offer chronic condition management programs.

For Medicare Plus BlueSM PPO and Blue Cross® Medicare Private Fee for Service members

The Blue Cross® Health & Wellness Chronic Condition Management program covers these conditions:

  • Coronary artery disease, or CAD
  • Chronic obstructive pulmonary disease, or COPD
  • Diabetes
  • Heart failure

If you're eligible, a registered nurse will call you to get started. Together, you'll create a personalized care plan. You'll set goals and build the skills you need to reach them.

If your condition needs extra attention, we might give you an in-home monitoring device. These monitors help detect changes in your medical condition and send health status alerts to your doctor. We'll teach you how to use it.

The program can also help you:

  • Take your medications safely
  • Get the medical tests and vaccinations you need
  • Communicate with your doctor and other health care professionals
  • Know your symptoms and respond to warning signs
  • Be more active and lose weight
  • Quit smoking

If you have a chronic condition, we'll automatically enroll you in the program. To start, you'll get a Lifestyles guidebook. Along the way, you'll get our newsletters in the mail, containing health, nutrition and fitness articles about your condition.
