Heart Failure
Heart failure happens when the heart muscle becomes weak. It doesn't pump your blood as well as it should to supply your body with oxygen and nutrients.
Our heart failure program is part of our chronic condition management program. We want to help you understand heart failure and learn how to live well with it.
Managing heart failure
Despite the name, heart failure doesn’t mean your heart is no longer working or could stop working soon.
By following your treatment plan, there’s a lot you can do to live well. You can manage your condition and work with your doctor to:
- Create a personal action plan (PDF)
- Use medicines correctly
- Weigh yourself daily and monitor your weight with the symptom tracker (PDF)
- Quit smoking
