Surgery. Serious illness. Conditions that are difficult to manage. We know these are some of the most stressful medical experiences for Blue Care Network members. So we give extra support when you need it the most with our case management and chronic condition management programs. It's all about getting help for getting better.
Blue Cross Blue Shield of Michigan members should check their plan to see if they're eligible for similar services. You can also call the Customer Service number on the back of your enrollee ID card for help.
Our chronic condition management programs
Blue Care Network offers chronic condition management programs for illnesses that can be challenging to deal with. If you're a Blue Care Network member, you can enroll in these programs. But anyone is welcome to read through the information for helpful tips and advice.

Before surgery
In some cases, if you're scheduled for surgery, you'll get a phone call from a nurse before surgery.
During this call, the nurse will discuss:
- What to expect while you're in the hospital
- Strategies for reducing post-operative complications
- A plan for your care following your discharge
If you're hospitalized
If you're admitted to the hospital, a nurse will follow your hospital stay and assist in coordinating care after you're discharged.
Quit the Nic
Our smoking cessation program has a proven track record of helping members give up tobacco for good. Quit the Nic members get their over-the-counter drugs covered for three months. This brochure (PDF) has more information about the program.
Stroke and heart attack prevention
We contact members who’ve had heart attacks to inform them of the importance of beta-blocker treatments.
