How do I know if I have to take the health assessment?

Your health care plan might require you to take a health assessment every year. You may also need to participate in the digital health coaching programs. Here's how you can find out.

• If you're a Blue Cross Blue Shield of Michigan member, check your benefit information to see if you're required to take a health assessment.
• Are you a member of Blue Care Network's Healthy Blue Living℠ or Healthy Blue Living Rewards℠ plans? Unless you scored all As on your qualification form, you'll need to take a health assessment to get intermediate or enhanced benefits. Details are in your member handbook.
• If you're a member of a different Blue Care Network plan, call the Customer Service number on the back of your enrollee ID card. We'll help you find out if you need to take the health assessment.
