How do I complete my health assessment?

Some of our plans ask you to take an annual health assessment. If you want to earn lower copays and deductibles from the Healthy Blue Living℠ and Healthy Blue Living Rewards℠ plans, it's required.

Even if your plan doesn't require it, a health assessment helps you improve or maintain your health. It's easy to do and doesn't take very long (around 20-30 minutes for most people). Here's how.

Where's the health assessment located?

You take your health assessment through your bcbsm.com online account. Just follow these steps.

1. To access your online account, select the login tab on the blue bar above or use this link. If you haven’t logged on before, you’ll need to register. Have your enrollee ID card handy so you can refer to it during the registration process. Each member on a contract must log in separately and create a unique ID and password.

Tip: Because your online security is important to us, answering some questions is part of the first-time registration procedure. If your answers don't match what we have in our system, you may have to wait for a PIN to arrive in the mail. The PIN is so you can see your personal information, like claims. You don't need your PIN to take your health assessment. You can login without the PIN.

2. Click the Health and Wellness tab.

3. Click My Health Assessment. It will take you to BlueHealthConnection®, where you can begin the Succeed questionnaire. That's the name of the health assessment.
