• Contract and member information shows your name, address and who received the services on the EOB. You’ll also see group and contract information from the claim.
  • Customer service information gives you a toll-free number and address you can use to contact us if you have any questions about your statement. You can also find this info on your Blue Cross ID card.
  • Claim Summary summarizes any claims we received from hospitals, doctors or health care professionals since your last EOB. You’ll see the charge(s) for the service, any savings you received by being a Blue Cross member, what we or another insurance carrier paid and what you paid or might have to pay.
  • Summary of Deductibles and Out-of-pocket Maximums shows you how close you are to meeting your deductible, coinsurance and out-of-pocket maximum for the specified benefit period.
  • Claim Detail has two sections. On the left, we show you what type of care you received, the date of your appointment and your doctor’s name. On the right, we show you the charge(s) for the service, payments already made (by Medicare or another insurer) and details about what you paid or might have to pay.
