Primary care can help keep people healthy and avoid expensive complications down the road. We're looking for innovative ways to help nonprofit health clinics to expand this type of care in underserved communities.
If your clinic serves the uninsured and underinsured and you’d like to apply for a grant, here’s what you need to do.
About the grants
We’ll award five grants, for up to two years each, for a total of $250,000. Program funds will be used for salary support, program costs, supplies and other costs related to the proposed project.
The grant is open to all free clinics, hybrid clinics, federally qualified health centers, look-alike health centers, rural health clinics and tribal health centers.
How to apply
To be considered for a grant, you'll need to submit a letter of interest by Feb. 14, 2014.
Please review the grant requirements and application form.
We'll review letters of interest by April 1, 2014. Then we'll select organizations to send in full proposals. The full proposals will be due May 1, 2014.
