How can I change the address or name on my policy?
Who is this for?
If you have an individual health plan or an employer-sponsored health plan, this information will show you how to change your name or address with us.
If you have changed your name or address, please let us know by filling out a change of status form. After you choose which form you want below, you'll find instructions on how you can send your changes to us.
Individual health plan members
If you have an individual health plan, meaning the coverage you buy directly from us, you'll need to fill out a change of status form:
- Blue Cross Blue Shield of Michigan Individual Billed Member Change of Status Request form
- Blue Care Network Individual Billed Member Change of Status form

Employer-sponsored health plan members
If you are enrolled in an employer-sponsored health plan, meaning the coverage you receive through your employer, fill out the Change of Status form.
