What if I don’t qualify for COBRA?

If you lose your employer-sponsored coverage and aren’t eligible for COBRA, we can help. You can switch from your employer’s coverage to an individual health insurance plan, called group conversion. You and your family can apply for individual coverage within 30 days of losing your employer’s coverage.

For information on the MyBlue℠ individual health insurance plans that might work for you under group conversion:

If you're applying for Blue Care Network group conversion coverage, you'll need to use the Nongroup/Group Conversion Enrollment Change of Status form (PDF). Fill it out and return it to Blue Care Network along with:

  • Written notification of coverage ending
  • First two months' premium payment, made payable to Blue Care Network
